4.6 Differential Price Regulations A fourth source of PI is that international price differences may stem from national price regulations established to achieve particular social objectives. The most prominent example arises in pharmaceuticals, a sector in which virtually all nations regulate prices in order to limit consumer costs or public health budgets. Evidence shows that such regulations vary considerably across countries and account for significant price variability Danzon, 1997 ; . Permission of parallel trade could defeat the purposes of regulation as distributors in more regulated lower-price ; markets ship medicines to less regulated higher-price ; markets. In turn, those exports could exacerbate any shortages in the export market of medicines arising from the regulatory standards. In this context, one argument for banning or controlling parallel exports in goods for which prices are heavily regulated stems from the need to defend the regulations. A second argument is that parallel imports coming from regulated markets could be restricted on the theory that such regulations amount to a sector-specific export subsidy Abbott, 1998 ; . This interpretation has not been tested at the WTO. While there is little evidence that PI flows actually do interfere with the ability to control prices see next section ; , one case provides such a stark example of these principles that it bears particular mention. The HIV AIDS crisis in Sub-Saharan Africa, now spreading to South Asia and Southeast Asia, can only be managed without further catastrophe if drug treatments are made available at very low cost to sufferers in those regions. Annex A of this report contains a working paper on this subject, in which I and two co-authors ; call for a policy that would combine public purchase of the patent rights in target countries and provision of medicines at very low cost. Equally important, there must be tight controls on parallel exports of these drugs out of the target countries and tight controls on parallel imports of these drugs from the target countries into rich nations. Such trade restraints would be necessary to support what would be beneficial price discrimination though organized in a different fashion ; in this case!

Can iraq be fixed?" [August 7] is a hard question to answer. Not if we stayed 100 years could we leave behind a stable, unified, secular Iraqi democracy of cooperating Shiites, Sunnis, and Kurds. Everyone knows this but the alchemists in the Bush administration. The best we might hope for is that Sunni battalions in the Iraqi Army control insurgency in Sunni areas, allowing a Sunni theocracy, and Shiite battalions do the same in their districts for their theocracies. Kurds, in any case, will continue their separate way toward eventual independence, hopefully not shattering U.S.-Turkish relations and glimepiride. Genetic basis medical liability the mean esgic diffusion, for instance, theophylline drip. Absorption in the intestines. The slow but complete drug release in the stomach is expected to increase bioavailability of the drug as well its complete utilization which may results to, lower dose and GI side effects. Multi unit dosage forms are considered to release the drug at a controlled rate and remain in the stomach for a prolonged period with much less chance of dose dumping. Furthermore they are supposed to cause less gastric adverse reactions and are insensitive to concomitant food intake, thereby reducing interand intra-patient variability and increasing the predictability of the dosage form 11-18 ; . A vast number of studies, reviews and books have been written on microspheres of which the interested readers are referred to compilations by Deasy 19 ; and Benita 20 ; for a broad overview of the dosage form for further information. In the recent literature, the Ethylcellulose microcapsules have been reported by several authors for encapsulation of a variety of drugs such as zidovudine 21 ; , cimetidine 22 ; , potassium chloride 23 ; , isosorbide dinitrate 24 ; , theophylline 25 ; , Isoniazid 26 ; , etc for a variety of reasons. The floating microspheres are relatively new compared to non-floating multiple unit systems. There are reports for such as repaglinide 27 ; , atenolol 28 ; , diclofenac 13 ; , terfenadine 17 ; , riboflavine 18 ; etc., which have been incorporated in floating multiple unit systems. Various novel excipients such as chitosan 12, 29 ; , calcium silicate 27 ; , low density foam powder 15, 16 ; besides the conventional polymers such as acrylic resins 30 ; and polycarbonate 14 ; have been used to achieve floatation . There are several excellent reviews on the gastro-retentive systems including floating dosage forms to which the interested readers are referred 31, 32, 33 ; . However, no floating microsphere of metformin has been reported. A non-floating multi-particulate metformin containing system has been reported in literature 34 ; , though the intention of the work was to optimize the pellets for extrusionspheronization purpose rather than to extend the drug release. There have been contradictory reports on the utilization of metformin in gastro-retentive dosage forms 2, 35 ; . However the investigated systems were single-unit type. Therefore, it seemed reasonable to improve the earlier studies by formulating metformin in a multiparticulate floating gastro-retentive ; system in order to optimize the pharmacokinetics and pharmacodynamics of the drug. Hence, to achieve the ultimate goal of formulating a clinically effective FDDS of metformin hydrochloride for effective control of Non Insulin Dependent Diabetes and acetaminophen.

Moderate. Drug Interactions: Analysis and Management assigned this interaction a significance rating of "3--minimize risk" on their 5-point rating scale. Because the severity ratings in the latter 2 references were somewhat equivocal in meeting the study criteria, the U.S. Food and Drug Administration FDA ; -approved prescribing information for sildenafil was used to verify the presence of the interaction. Similarly, the absence of an interaction for the remaining drug pairs was evaluated by verifying the absence of the abovementioned criteria for each drug pair. Two drug pairs that were not considered DDIs met the criteria. The azithromycin and digoxin combination was listed as a "major" DDI by Evaluation of Drug Interactions and as a moderate interaction by DRUGREAX Micromedex. In both cases, the accompanying monographs referred to the interaction between erythromycin and digoxin and acknowledged the lack of information on azithromycin. The remaining references did not list the interaction; in addition, the prescribing information for azithromycin did not include an interaction with digoxin. Azithromycin and theophylline was listed as a moderate interaction by Drug Interaction Facts and DRUG-REAX Micromedex. Drug Interactions: Analysis and Management specifically listed it as "No interaction"; Evaluation of Drug Interactions did not mention it. Again, both monographs referred to the interaction between erythromycin and theophylline and acknowledged that a pharmacokinetic study showed no significant effect of azithromycin on theophylline levels. In addition, the prescribing information for azithromycin states that no dosage adjustment is necessary when coadministering it with theophylline. Given that both these combinations were included in the Hazlet et al.9 study as noninteractions and there were sufficient data to substantiate that claim, they were included as noninteractions in the present study as well. One investigator visited each participating pharmacy to collect data on the performance of the pharmacy's DDI software. Each patient profile was entered into the pharmacy system by the investigator or by a pharmacy staff member while the investigator observed. The pharmacy computer system's response was assigned a severity level if either an alert was generated or there was no alert for each drug pair; it was recorded on a standard data collection form. The response was also printed whenever possible. Two exceptions to this process occurred in 1 community and in 1 hospital pharmacy. One community pharmacy did not allow the investigator to be on-site during testing so it was performed by the pharmacy manager and the results were sent to the investigators. In the case of the hospital pharmacy exception, the investigators obtained a comprehensive list of all the DDIs and corresponding severity ratings that are used by the hospital pharmacy system and assessed the performance using this information. The percentage correct was calculated for responses from community and hospital pharmacies for each individual drug.

It was possible to calculate the DDD PDD ratios for 15 of 18 asthma COPD drugs. For three drugs -- the short acting adrenergics salbutamol R03AC02 ; and terbutaline R03AC03 ; and the combination product of salbutamol and ipratropium R03AK04 ; -- over 95% were prescribed to be used "when needed". No DDD PDD ratios were therefore determined for these drugs. For the youngest age group, 2029 years, the number of prescriptions for 9 of the drugs was too low to make a determination of the DDD PDD ratios meaningful. PDDs were obtained from the pharmacy record data base of the same county. Data on R03 drugs from this data base 11 ; have previously been determined to correlate closely with data from the national register 8 ; . The ratios are likely to be representative of the current situation in the rest of Sweden. The DDD PDD ratios for most drugs showed only small variations between the different age groups. For fluticasone there was a two-fold variation among the age groups with higher ratios for young and elderly patients. The reason for this is not clear. For ipratropium bromide the ratio increased by age. It is apparent that doctors prescribed lower doses of this anticholinergic drug to older patients, although no such recommendation is included in the specifications for the drug 12 ; . Of drugs, the mean DDD PDD ratios were close to 1 for 10. Two drugs, formoterol for inhalation and terbutaline for oral use, had DDD PDD ratios of 0.7 and 0.6, respectively, showing that 1.4 and 1.6 DDDs were the average prescribed doses for these drugs. For three other drugs, fluticasone, ipratropium bromide, and theophylline the ratios were 1.7, 2.7 and 1.4 respectively, indicating that the average prescribed dose for. The UWO Urologic Oncology Fellowship Training Program recently became accredited through the Society of Urologic Oncology. This is considered a significant accomplishment as there are only 10 other training programs in North America that have been accredited, putting our program in the company of major cancer institutions such as M.D. Anderson in Houston, the National Cancer Institute in Bethesda and Memorial Sloan Kettering Cancer Center in New York. The accreditation, under the direction of J. Chin, J. Izawa and S. Pautler, recognizes that the Urologic Oncology Fellowship Training Program fulfils the clinical, basic science and translational research components that are deemed to be necessary for this additional oncologic training for urologists wishing to pursue an academic career. The fellowship-training program is comprised of clinical rotations Uro-oncologic Surgery, Medical Oncology, Radiation Oncology, GU Pathology and Gynecologic Oncology, as well as a year of basic or translational research. Being successful in this vigorous accreditation process will allow us to attract the top candidates worldwide for our program.
