In outline, IDIS consists of parties from a broad range of disciplines and agencies. These may include drug treatment centres, law enforcement agencies and health services in their various forms. The task of IDIS is to gather and summarize the available information on drug use in a coherent and standard manner. The overriding goal is to monitor accurately the scale and nature of and trends in drug use in a country or region and thus improve the effectiveness of drug demand reduction responses. The need for support in basic data analysis has been identified as part of the Information, Needs and Resources Analysis INRA ; completed by GAP in Africa, the Caribbean and Central Asia. The present GAP Toolkit Module 5 is intended to meet, at least in part, this identified need, for instance, testosterone tablets. These studies compared the protective capacity of commonly used antioxidant ingredients in both in vitro and in vivo methods. Correlation and trends between the study results obtained allows the establishment of a standardization testing protocol that quantifies the oxidative-stress-protection capacity of the substances studied. It interacts with pituitary steroid receptors and decreases gonadotrophic secretion resulting in reduced follicular growth and anovulation. It has a longer half life than Danazol and thus can be taken twice weekly. Gonadotrophin-releasing hormone analogues GnRH ; GnRH suppresses production of pituitary gonadotrophins. Ovarian function and menstruation are almost completely arrested by almost complete suppression of FSH and LH. Side effects are much less common, eg. flushing, vaginal dryness, but long term use may result in loss of bone mineral density from prolonged oestrogen deficiency. Regimens called `addback' may overcome this; supplementary doses of oestrogen with GnRH may prove to be effective in treating symptoms without the complications of total oestrogen deprivation. GnRH analogues include buserelin nasal spray ; , goserelin subcutaneous implant administered into anterior abdominal wall ; , leuprorelin subcutaneous or intramuscular injection ; , nafarelin nasal spray ; and triptorelin intra muscular injection ; . After an initial short phase during which these drugs stimulate pituitary secretion, the output of pituitary gonadotrophins is inhibited and so ovarian function is suppressed. On current evidence it is not possible to state which is a `better' outcome of management. For relief of painful symptoms medical treatment may have an advantage in that some drugs can be used longterm, eg. medroxyprogesterone acetate and COCP. Potential unwanted side effects make danazol unsuitable for therapy beyond one year. Medical treatment may suit those many years before they wish to conceive. In patients with subfertility, hormone therapy with progestogens is beneficial, while certain forms of surgery may remove physical barriers and may improve fertility, eg. adhesions. Due to its varied aetiology, endometriosis has a range of treatment options, making management a challenge. In recently proposed medical hypothesis, Hayward, Webb, and Collins suggested increased left ventricular mass to be linked to endogenous hormone concentrations 33 ; . The authors suggest that normal decline in endogenous sex hormones with age has contrary effects on ventricular mass in men and women, particularly as androgens have an anabolic effect on cardiac cells, whereas estrogens have antiproliferative properties. There is speculation and circumstantial evidence that woman are protected against cardiac disease at least in part by higher estrogen hormone levels. Our study is the first report on testosterine metabolism in human normal, hypertrophic, and assist devicesupported heart tissue and we propose an important role for cytochrome P450 mono-oxygenases in cardiac physiology and heart disease. Further evidence comes from a study by Xiao et al. where modulation of cardiomyocyte contractility and L-type calcium channel was achieved with the potent CYP mono-oxygenase inhibitor and antifungal remedy imidazole 18 ; , and systemic therapy of patients with this antifungal has been linked to severe arrhythmias 58 ; . The CYP inhibitor clotrimazole was also reported to inhibit the sarcoplasmic reticulum calcium ATPase, leading to imbalance in calcium homeostasis and impaired cardiac contractile function 59 ; . In recent study by Ahmad et al. negative inotropic effects are reported for patients receiving systemic treatment with itraconazole 60 this provides further evidence for the important role of cytochrome P450 in cardiac physiology. In conclusion, we show metabolism of testosgerone to be significantly altered in hypertrophic hearts and we propose a novel role of CYP mono-oxygenases in cardiac hypertrophy via modulation of tissue-specific testosterone metabolism and hormone-dependent gene expression.
