In a large-scale study commissioned by CMS, 160 residents in 40 nursing homes were interviewed about what quality of life meant to them. The study found that residents "overwhelmingly assigned priority to dignity, although they labeled this concern in many ways." The researchers determined that the two main components of dignity, in the words of these residents, were "independence" and "positive self-image." This problem raises complex issues related to identity certification, which in itself might be a concept that our mistrustful drug dealers unconditionally reject a priori. Here we will merely point out a major practical pitfall. Suppose that some acceptable technical mechanism has been devised to create a certification authority. Spasticity is one of the most common MS symptoms, causing significant problems in approximately 40% to 60% of patients.1, 2 Spasticity results in obstructed functional mobility, and is a leading cause of disability in MS. Spasticity is a velocity-dependent state of increased muscular tonus associated with exaggerated tendon jerks and hyperexcitability of the stretch reflex.3, 4 The clinical manifestations of spasticity include phasic spasticity, which is associated with spasms, cramps, and clonus, and tonic spasticity, which produces stiffness.5 While the exact cause of spasticity is unknown, it may be due to demyelination disrupting conductivity in hyperexcitable spinal alpha-motor neurons in regions of the CNS that regulate muscle tone, including the corticospinal, vestibulospinal, and reticulospinal tracts.3 The muscle groups usually affected by spasticity are those most important for maintaining upright posture, walking, sitting and sleeping, located primarily in the calf, thigh, buttock, groin, and back.6, 7 The resultant loss of balance and difficulty in mobility, sitting, and sleeping means that more energy must be exerted to perform daily activities, increasing the fatigue.
